THE OPPORTUNITY:
The Heart and Stroke Foundation of New Brunswick is looking to hire a Stroke Navigator, on a 1-year contract (with the possibility of extension), to work within our Health Promotion team.
The Stroke Navigation Program is funded by the Government of New Brunswick, and is a community-based initiative designed to help people with stroke adjust to life back home. As a Stroke Navigator, you will work with clients individually to provide support, education, guidance, and resources for stroke recovery in the community.
The Stroke Navigator works as a non-clinical professional, who uses a combination of health promotion techniques, individualized guidance, and community resources to enhance the quality of life for their clients. The Stroke Navigator will support the community reintegration pillar of the Stroke Continuum of Care (https://www.strokebestpractices.ca/ ).

RESPONSIBILITIES:
Provide personalized stroke navigation to clients of the Stroke Navigation program to support quality of life, continuity of care, and stroke recovery.
Provide appropriate referrals for stroke patients to meet their unique needs
Act as a resource for any questions/concerns from both clients and/or public inquiries.
Carry out client recruitment and program promotion
Build relationships and expand networks with stakeholders & partners at a provincial level to enhance community connections and increase program participation.
Write and submit regular reports on programs
Make & execute decisions using a strategic lens
Assist with the organization of the annual New Brunswick Provincial Stroke Conference
Positively promote and maintain the vision, mission, and policies of the Foundation and the people it serves and supports
Undertakes special projects and other tasks as assigned
IDEAL QUALIFICATIONS:
Experience working with health-related or non-profit groups
Experience in sales
Bilingualism preferred but not essential
Valid license and the ability to travel as required
